Comments:
A Subsidy Health and Safety inspection was conducted on 4/30/2018 to review supplemental health and safety requirements. Children and staff files were reviewed, along with various documentation. There were 12 children, aged 3 years-5 years, in care with 2 staff. Children were active in group time, personal hygiene, outdoor time, and lunch time. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Standard #: VENDSUB-000-012-B
Description: Based on review of staff files and discussions with staff the vendor failed to maintain compliance with this standard as required relating to Pre-service training. Evidence: Staff #2, hire date 10/04/2017, had not yet completed the required 10 hour training. Staff have 90 days from date of hire to complete this on-line training.

Plan of Correction: Staff will complete this training within the allowed 90 days moving forward. Staff #2 will complete this as soon as possible.
