Comments:
An unannounced subsidy health and safety inspection was conducted on April 17, 2017 from approximately 3:00 pm to 6:30 pm to review supplemental health and safety requirements. Seven staff were present with fifty-four children. Additional staff were on site for administrative duties. Eight staff records and six children's records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Please complete the columns for ?description of action to be taken? and ?date to be corrected? for each violation cited on the violation notice, and then return a signed copy to the licensing office within 5 calendar days of receipt.

Standard #: VENDSUB-000-005-B
Description: Based on observation and review, vendor did not ensure there is a written record of daily attendance that documents the arrival and departure of each child in care as it occurs for each group of children. Evidence: On April 17, 2018, the group that included kindergarten, first and second grade had twenty-nine present in the room however the attendance sheet documented twenty-five children were present.

Plan of Correction: Staff will be retrained on keeping attendance documents accurate and timely.

Standard #: VENDSUB-000-012-B
Description: Based on review, vendor did not ensure 1 of 8 staff complete Virginia Preservice Training for Child Care Staff within the first 90 days of employment. Evidence: On April 17, 2018, the record for staff #4 (start date August 31, 2017) did not have documentation of the individual completing the Virginia Preservice Training.

Plan of Correction: Staff #4 will have Virginia Preservice Training completed and documented.

Standard #: VENDSUB-000-012-E-1
Description: Based on review, vendor did not ensure 2 of 8 staff have current CPR certification within 30 days of beginning employment. Evidence: On April 17, 2018, the records for staff #4 (start date August 31, 2017) and staff #5 (start date September 18, 2017) do not have documentation of current CPR certification.

Plan of Correction: Staff #4 and #5 will have CPR certification completed and documented.

Standard #: VENDSUB-000-012-E-2
Description: Based on review, vendor did not ensure 2 of 8 staff have current first aid certification within 30 days of beginning employment. Evidence: On April 17, 2018, the records for staff #4 (start date August 31, 2017) and staff #5 (start date September 18, 2017) do not have documentation of current first aid certification.

Plan of Correction: Staff #4 and #5 will have first aid certification completed and documented.
