Comments:
4/20/2018 Arrived 9:43am Departed 12:45pm An unannounced renewal inspection was completed today. One hundred eleven (111) children were in care with 19 staff. The age range of children in care is 6 weeks to five years. Children were having group time, playing in centers, eating lunch and resting. One group of children went on a field trip. Previous violations, medications, 10 children's files and seven staff files were reviewed. Thank you for your cooperation. Please contact me if I may be of service. 276 608 3749

Standard #: 22VAC40-185-70-A
Description: Based on the review of seven staff records, the facility failed to obtain references on all staff. Evidence: 1. There are no references on file for staff #3. (hired 3-20-18)

Plan of Correction: (2) References called & on file.

Standard #: 22VAC40-185-270-A
Description: Based on a tour of the playgrounds, the facility failed to insure that all areas and equipment are safe. Evidence: On the large climber: 1. On the beige panel with the window, the plastic is cracked and presents pinch points. 2. On the blue panel with two windows, one of two bolts is not secure and is hanging loose from the panel. 3. On the vertical blue and yellow plastic panels, there are many cracks in the plastic presenting pinch points and sharp edges. 4. On the top deck, the blue panel with plastic windows is missing two bolts. The blue plastic under the bottom window is cracked and presents pinch points. 5. In the beige tower there are four bolts that each have more than two threads showing presenting entanglement hazards. On the toddler playground there are large gravels which present choking hazards.

Plan of Correction: 1., 3., 4. Panels and parts which present potential pinch hazards will be repaired or replaced. 2., 4., 5. All bolts will be secured; missing bolts will be replaced. Large gravel will be raked off the toddler playground to eliminate choking hazards.

Standard #: 22VAC40-185-330-B
Description: Based on observation, the facility failed to insure that resilient surfacing shall be under equipment with moving parts or climbing apparatus. Evidence: 1. On the toddler playground there are six plastic slides. There is no resilient surface material under these slides.

Plan of Correction: All slides will be removed until resilient surfacing is installed.
