Standard #: 22VAC40-185-130-A
Description: Based on a review of children's records and staff interview on 04/26/2018, the center did not ensure that documentation that each child has received the immunizations required by the State Board of Health were obtained before the child can attend the center. EVIDENCE: 1. The record of Child #5 did not contain documentation of immunizations. 2. Staff #1 stated that Child #5 started at the center on 03/19/2018.

Plan of Correction: The physical and immunizations were located on the home office. Will be in the child's record by the end of the day today.
