Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted from 11:00 am - 1:15 pm. There were 100 children in care being supervised by 15 staff members. The children were observed participating in structure center play, circle time activities, and preparing for lunch. Lunch served today is turkey and cheese sandwich, corn, pears, and milk. Eight children's records were reviewed. Eight staff records were reviewed. An allergy list was posted for children enrolled in the program with diagnosed food allergies. Training was reviewed for CPR, First Aid, and Virginia Pre-Service Training completion. Two medications were reviewed. Description: Based on review of eight children's records and interview with administration it was determined that not all children enrolled in the program with diagnosed food allergies had an allergy care plan on file. Evidence: The following children with diagnosed food allergies did not have an allergy care plan on file: Child A, B, C, D, E, F, and G.

Plan of Correction: Administration will request an allergy care plan from the parents of child A, B, C, D, E, F, and G.

Description: Based on review of staff training it was determined that not all staff obtained documentation of Virginia Pre-Service Training within the first 90 days of employment. Evidence: Staff A with a start date of 09/11/2017 did not have documentation of Virginia Pre-Service Training on file.

Plan of Correction: Staff A will complete Virginia Pre-Service Training.
