Technical Assistance:
The essential documents needed for children's records was discussed with the licensee.

Comments:
An unannounced Monitoring Inspection was conducted on April 30, 2018 from approximately 4:00 pm until 5:35 pm. Upon entrance there were 24 children and 3 staff present with children. The record review sample consisted of 5 children's records and the emergency information for 3 staff. One medication was reviewed. Children were observed playing board games, Legos, working on homework, having snack and practicing a fire drill. The areas of non-compliance are noted on the violation notice. The violations were reviewed with the director during the exit interview. Please contact the licensing inspector with any questions (757) 404-0568.

Standard #: 22VAC40-185-60-A
Description: Based on a record review of 5 children's records and staff interviews, the licensee did not ensure the center shall maintain and keep at the center a separate record for each child enrolled which shall contain the required information. Evidence: During the inspection on April 30, 2018 2 of the 5 records were determined incomplete as listed below; 1) The record for child #1 did not include the one parents' place of employment, the employment phone number and the home phone number, state and zip code for the second parent listed. 2) The record for child #2 did not include the city, state and zip code for the second parent listed.

Plan of Correction: The center director will have parents update the records.

Standard #: 22VAC40-185-510-L
Description: Based on a review medication , medication related documentation and staff interviews, the licensee did not ensure the center shall keep a record of medication given to children. Evidence: During the inspection on April 30, 2018, there was 1 medication on site and it was reviewed. Center director confirmed the medication had been administered in January 2018 and there was not a written record of administering the medication.

Plan of Correction: The center director will keep medication administered log in another location from the medication.
