A complaint was received on October 6, 2017 for an incident which occurred on September 8, 2017 concerning lack of supervision. The complaint was initiated on October 12, 2017. An on-site investigation was conducted on 12/11/17. The investigation was concluded 4/16/18. The information gathered during the investigation supports the allegation. Therefore, the complaint is found to be valid. One violation concerning supervision was sighted and may be reviewed on the violation notice.

Description: Based interview and view of the playground the center failed to demonstrate that when staff are supervising children they always ensure their care and protection. Evidence: Child 1 sustained a scrape and bump on the head while riding a tricycle on the center playground. 1 ? Staff 1 reports that the ?children had a made a game of riding a bike down a mulch hill on the playground.? 2 - When child 1 started down the slop staff 1 stated ?she knew what was coming.? As she was moving toward the child she saw ?child 1 go over the trike handle bars and land on ground on his forehead. 3 ? Staff failed to intervene before the injury occurred. 4 ? Children were allowed ride a tricycle down a mulch hill. Children were not directed to use the tricycle on the flat and paved surface provided on the center playground.
