Comments:
An unannounced monitoring inspection was conducted today from approximately 9:40am-11:50am. There were 105 children in care directly supervised by 22 staff. A sample of 10 children?s records, 7 staff records, and 5 medications were reviewed. The children were observed playing, reading, on the playground, doing crafts, and telling stories. Zero new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the director. Violations:
Standard #: 22VAC40-185-80-A
Description: Based on observation and interview with staff, a written record of daily attendance was not kept. Evidence: In room 117, the attendance for Friday April 27th was not recorded. Staff stated that they were open on Friday and all children were present.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-185-270-A
Description: Based on observation, equipment of the center, inside and outside, was not maintained in a clean, safe and operable condition. Evidence: Chipping paint was found on the swing poles located on the playground.

Plan of Correction: The poles will be painted again during the summer months.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not kept in a locked place. Evidence: 1) In room 112, hand sanitizer labeled "keep out of reach of children" was found on the counter near the sink and on top of cubbies labeled with the children's names. 2) In room 125, hand sanitizer labeled "keep out of reach of children" was found on a shelf above the children's backpacks. 3) In room 125, disinfectant wipes were found in an unlocked closet.

Plan of Correction: Corrected during inspection.

Description: Based on observation, food brought from home was not clearly labeled with the child's name and date. Evidence: 1) In room 127, 1 lunch box was not labeled with the child's name and date. 2) In room 125, 1 lunch box was not labeled with the child's name and date. 3) In room 124, 4 lunch boxes were not labeled correctly, 3 did not have a date and 1 did not have a name and date. 4) In room 121, 1 lunch box was not labeled with the date.

Plan of Correction: Parents will be reminded in the next news letter and teachers will be reminded to inspect lunch boxes for labels upon arrival.
