Discussed with Director: staff moving children to maintain ratios while administrative staff are available.
An unannounced monitoring inspection was begun in response to a self reported incident at the facility. The investigation was conducted jointly with local authorities. Based upon a review of available documentation and multiple staff statements, the preponderance of evidence supports a violation. Other violations cited were discovered during the course of the investigation. Standard #: 22VAC40-185-40-E Description: Based upon multiple staff statements, staff ailed to follow the center policy of conducting a name to face check whenever you cross a threshold. Evidence: Staff failed to conduct a name to face check as infants were placed in the buggy outside of the classroom. One infant was left in the classroom, asleep in a crib as a result of failing to conduct the check. Plan of Correction: Staff will be retrained on center policy for Transition Tracking procedures which includes a face-to-name check. Licensing compliance manager will remain in place to double check face-to name checks when children leave a classroom.
Standard #: 22VAC40-185-340-F Description: Based upon multiple staff statements, one infant was not kept within actual sight and sound supervision. Evidence: Child #1 (age 9 months) was not kept in actual sight and sound supervision when staff were taking the children from the infant room to the playground. The child had been placed in a crib, as staff were preparing to exit the room for the playground. The other infants in the room were placed in a buggy, in the hallway of the facility. Child #1 was left sleeping in the room as the group went down the hall to exit the building. An Administrator performing a second name to face check discovered the child was not with the group. Staff B returned to the room to retrieve the child from the crib. Plan of Correction: Staff will be retrained on center policy for Transition Tracking procedures which includes a face-to-name check and prevents children from being without sight and sound supervision. Staff will also complete Supervision Training again. Licensing compliance manager will remain in place to double check face-to-name checks when children leave a classroom.
Standard #: 22VAC40-185-440-B Description: Based upon staff interviews, a child was placed in a crib specifically assigned to another child. Evidence: Child #1 was placed into Child #2's crib when she was "tired and fussy" as the room was preparing to go outside to the playground. Staff stated the crib was closer to the front of the room, nearest the door. Plan of Correction: Staff will be retrained on both Healthy Infant Environments and Safe Sleep Practices.
Standard #: 63.2-1720.1-B-3 Description: Based upon a review of records, one staff record does not contain documentation of an out of state child abuse registry search for someone residing outside of the state in the past 5 years. Evidence: Staff A resided in Maryland within the past 5 years. There is no documentation of an out of state search having been submitted. Plan of Correction: In addition to the NJ and VA checks already completed for Staff A, a Maryland child abuse registry search will be completed and sent immediately. Old Sworn Disclosure statements will be reviewed for any missed states of residency for all staff members.
