22VAC40-191 Background Checks (22VAC40-191)

An unannounced complaint investigation was conducted and found to be not valid-no violations relating to the allegation were cited. An unannounced monitoring was also conducted on the same day from 12:15-2:15. All rooms were in ratio with direct care staff. Observed physical plant, activities and transitions to rest time and lunch (mac and cheese, green salad, oranges and milk), medication review and emergency preparation. Playground was not observed today due to the wet weather. Areas of non-compliance are identified in the violation notice. Description: Based on record review, a TB screening result has not been obtained for a staff member. Evidence: Staff #3's TB results expired 11/2017 and has not been repeated within 2 years of the expiration date.

Plan of Correction: Staff member will be given form to obtain results.

Description: Based on record review, 1 of 2 new staff did not have a second reference check prior to employment. Evidence: Staff #1 was hired 4/16/2018 and had one of two required references.

Plan of Correction: Center management will request a second reference.

Description: Based on orientation documentation and center management statement, 2 of 2 new staff have not received orientation training by the end of the first day of assuming job responsibilities. Evidence: Staff #1 started 4/16/2018 and Staff #2 started 4/11/2018 in the classrooms and have not completed the orientation training as required.

Plan of Correction: Center management will update orientation training.

Description: Based on medication review, a medication for child #1 was expired. Evidence: Child #1's medication expired 3/2018 and has not been replaced with an up to date medication.

Plan of Correction: The medication will be sent home to the parent tonight and a new one will be requested.
