Technical Assistance:
An exit interview was conducted with the director to discuss the results of today's inspection. Consultation was given on updated background check requirements.

Comments:
An unannounced monitoring inspection was conducted today, April 30, 2018 to determine the center's compliance with licensing standards. The inspection was initiated at 3:15pm and concluded at approximately 5:30pm. Upon arrival, two toddlers and eleven Pre K children were observed being supervised by two staff. Children were observed playing outside, reciting nursery rhymes, and participating in organized educational games. Departure procedures were also observed on this day as parents signed their children out for the evening. All areas used by children were inspected to include the main hallways, children's restrooms, classrooms, and outdoor play areas. The center had the required postings displayed on the wall upon entering the building. The center's first aid kit and emergency supplies were inspected with extra supplies found. Seven children's records and two staff records were inspected with the majority of the required information present. The following was also inspected on this day: annual fire & health inspections, fire/shelter-in-place drills, parent agreements, and other pertinent information. Standard #: 22VAC40-185-160-C
Description: Based on the inspection of administrative records, the required documentation was not present to review. Evidence: During the inspection of administrative records, it was discovered staff #1 did not have documentation of the most recent tuberculosis (TB) test received. Licensing standards require updated TB test or screenings to be obtained every two years from the date of the initial test with documentation maintained for review.

Plan of Correction: Per director: I will have the staff to obtain a copy of the last TB test from the doctor's office.

Standard #: 22VAC40-185-240-C
Description: Based on the inspection of administrative records, the required annual training hours were not obtained.

Evidence:
During the inspection of staff records, it was discovered staff #1 had 10 hours of annual trainings for the year 2017. Licensing standards require staff to obtain a minimum of 16 hours of training in child related subjects each year.

Plan of Correction: Per director: I will have the staff to take additional training and make sure staff have the required number of trainings each year.
