The licensing inspector completed an inspection in response to a complaint regarding supervision and behavior guidance that occurred at the facility on 04/09/18. The Department was notified on 04/12/18. The licensing inspector was on site at the facility on 04/17/18 from approximately 1:20 PM to 3:40 PM. During the inspection, interviews with staff were completed, documentation was reviewed and classroom observations were conducted. Additional interviews were completed on 04/25/18. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, there were violations found during the inspection regarding staff records. See the violation notice for more details. Please complete the 'plan of correction' and 'date to be corrected' for each violation cited on the violation notice and return it to me within 5 business days from date of receipt. You will need to specify how the deficient practice will be or has been corrected. Just writing the word 'corrected' is not acceptable. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s). Standard #: 22VAC40-185-160-A Complaint related: No Description: Based on the review of five staff records, one staff member did not obtain documentation of a negative tuberculosis screening within the required time frame. Evidence: During the inspection, the licensing inspector observed the record for Staff #2 (hire date 01/02/18) contained a negative tuberculosis screening dated 01/30/2018. Documentation of a negative tuberculosis screening is required to be completed with 12 months prior to or within the first 21 days of staff beginning employment. Plan of Correction: We will request new employees have their TB screening complete before their first day of employment . Monitoring will be done by the assistant director.
