22VAC40-191 Background Checks (22VAC40-191)

Technical Assistance:

The licensing inspector conducted an unannounced monitoring inspection Thursday, May 3, 2018 from approximately 10:20AM to 2:00PM. The following ratios were observed with a total of 43 children present during the inspection: Infants - 7 children with 2 staff Toddlers and Twos - 12 children with 3 staff Preschool - 13 children with 2 staff Pre-K - 15 children with 2 staff School age children were not present during the inspection. During the inspection children were observed playing with toys, sleeping in cribs, having their diapers changed and taking bathroom breaks and washing hands, having books read to them, playing outside, eating lunch and resting at naptime. All areas of the center including classrooms, hallways, kitchen, bathrooms and playgrounds were inspected. The facility was equipped with toys and supplies and items were available to the children. The following information was reviewed: injury reports, injury prevention plan, daily sheets, daily attendance, practiced emergency drill logs, parent agreements, staff training information and liability insurance. The center's first aid kit and emergency supplies were inspected. Medication is administered and medication and medication authorization forms were reviewed. Last documented health inspection: 04/30/2018 Last documented fire inspection: 01/25/2018 During the inspection, five children's records and five staff records were reviewed. Standard #: 22VAC40-185-60-A
Description: Based on review of five children's records, one record did not contain the required information. Evidence: The record for Child #1 (enrollment date 04/02/2018) did not contain the address for one of the two emergency contacts listed in the record.

Plan of Correction: I have already let the parent know the second address was needed. I will let her know again today that I need the address and add it to the record.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not ensure all areas were maintained in a safe and operable condition. Evidence: During the inspection, the licensing inspector observed a hole in the wall in the Infants and Preschool classroom with exposed dry wall.

Plan of Correction: Corrected during the inspection.

Standard #: 22VAC40-185-330-B
Description: Based on observations and measurements, the facility did not ensure the required resilient surfacing was located in the required fall zones. Evidence: During the inspection of the playgrounds, a plastic slide that was approximately three and a half feet tall had approximately three inches of pea gravel at the end of the slide chute. Six inches of pea gravel would be required in all fall zone areas around the slide.

Plan of Correction: We will have additional gravel added by this weekend.
