Standard #: 22VAC40-185-60-A Description: Based on review of five children's records, one record did not contain the required information. Evidence: The record for Child #1 (enrollment date 04/02/2018) did not contain the address for one of the two emergency contacts listed in the record. Plan of Correction: I have already let the parent know the second address was needed. I will let her know again today that I need the address and add it to the record.
Standard #: 22VAC40-185-270-A Description: Based on observation, the center did not ensure all areas were maintained in a safe and operable condition. Evidence: During the inspection, the licensing inspector observed a hole in the wall in the Infants and Preschool classroom with exposed dry wall. Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-185-330-B Description: Based on observations and measurements, the facility did not ensure the required resilient surfacing was located in the required fall zones. Evidence: During the inspection of the playgrounds, a plastic slide that was approximately three and a half feet tall had approximately three inches of pea gravel at the end of the slide chute. Six inches of pea gravel would be required in all fall zone areas around the slide. Plan of Correction: We will have additional gravel added by this weekend.
