Technical Assistance:
During the inspection, the licensing representative discussed with center administrators staff records, tuberculosis screening [Staff 2] and central registry results [Staff 4] that will be needed for recent hires in the near future. The center has received their facility code and has began the process of obtaining fingerprint results according to the new background regulations.

Comments:
An unannounced monitoring inspection was conducted today from 11:00am-1:50pm. A total of 58 children were observed in the care of 10 staff. Outdoor play, infant bottle feeding, handwashing, lunch [which consisted of rice, beans, vegetables, and fresh strawberries] were observed. A sample of 6 staff records, 5 children's records, medications, emergency drills and other documents were reviewed. Areas of noncompliance are noted on violation report. Standard #: 22VAC40-185-160-A
Description: Based on a review of records, 1 staff record was missing results of a negative tuberculosis screening within 21 days of employment. Evidence: The record for Staff #6 [date of employment: 2/5/2018] was missing results of a tuberculosis screening.

Plan of Correction: We will add the results to the record.

Standard #: 22VAC40-185-70-A
Description: Based on a review, staff records did not include the required information. Evidence: 1. The record for Staff #5 was missing emergency contacts

Description: Based on a review, 1 staff record was missing documentation of the required training. Evidence: The record for Staff #1 was missing documentation of orientation training by the end of their first day of employment [3/16/2018]

Description: Based on observation, medications were not kept in a locked place. Evidence: Medications observed in Classroom B and G were stored in a bookbag in an unlocked upper cabinet.

Description: Based on observation, requirements for the use of diaper ointments were not met. Evidence: Written authorizations were not provided for the use of 8 out of 12 diaper creams in Classroom F.

Description: Based on a review, 1 staff record was missing central registry search results from another state in which the individual has resided within the past five years. Evidence: Staff #6 disclosed on a sworn statement residing in the state of Maryland; however, the record was missing a request of central registry results from that state.
