Licensing Inspector arrived at 9:00 and departed at 11:30 AM to conduct a monitoring visit. At the time of the inspection, there were 121 children (ages infant through four years) in care with 20 staff in the station, 17 school-age children with three staff in the Village and three children in the Get Well Place with two staff. Children were observed in participating all types of preschool activities, indoor and outdoors. Violations were noted in the areas of administration, physical plant and special care provisions & emergencies.
Standard #: 22VAC40-185-130-B Description: Based on review of ten children's records, it was determined that the facility did not ensure that the center obtained documentation of additional immunizations once every six months for children under the age of two years. Evidence: The following children needed updated immunizations: - Child #1 - last immunization 10/11/17 - Child #6 - last immunization 11/4/16 - Child #7 - last immunization 3/7/16 Plan of Correction: Facility is redoing their procedure to have Administrative Assistant follow-up on immunizations instead of current system.
Standard #: 22VAC40-185-270-A Description: Based on observation, it was determined that the facility did not ensure that all areas of the center were maintained in a safe operable condition. Evidence: There were areas on the top support bar of the chain link fence on the playground that had areas of peeling paint. Plan of Correction: Facility management will ensure that top support bar of fence is repainted.
Standard #: 22VAC40-185-510-C Description: Based on review of medication, it was determined that the facility did not ensure that long term medication included a physician's signature. Evidence: There was an epi-pen stored in the medication cabinet for child #1. Although there was signed parental approval, there was no physician signature available. Plan of Correction: Facility management will ensure that the nurse obtains the physician signature from the parent.
