Comments:
An unannounced monitoring inspection was conducted today from 9:15am to 11:30am. 64 children were directly supervised by 12 staff members. Ratios were in compliance. The physical space, 5 staff records, 7 children's records, evacuation drills, injury reports, allergy reports, the menu and attendance records were reviewed. The diapering procedure was observed and infant bottles were properly labeled. Required documents were posted. Children were observed working in small group activities, working on art projects, listening to stories and interacting with staff members. Interactions were positive. Children had an abundant supply of materials to choose from in each classroom. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-270-A
Description: Based on inspection, a bathroom across from the Toddler's room was not maintained in a safe and operable condition. Evidence: The tile floor is cracked and the drain is protruding out of the floor. The drain is located in the center of the floor, in close proximity to two toilets. The drain and the cracked tile are a tripping hazard.

Plan of Correction: The floors will be repaired by May 13th. (The school owner contacted the building owner while the Licensing Inspector was present. The school owner also produced emails to the building owner requesting the floor to be repaired after the last inspection.)
