Technical Assistance:
Discussed medication - make sure the parents write when medication should be given.

Comments:
An unannounced inspection was conducted from 930- 11:00. The Inspector observed classes, ratios, and program. The staff were aware of the children, care of the children, and ratios. The Inspector checked files, reviewed evacuation drills, menu, MAT certificates, and CPR/First Aid cards. Standard #: 22VAC40-185-60-A
Description: Based on record review, children's files were incomplete. Evidence: 1. Child #1's file does not include start date and has listed various food allergies but does not have a plan of action in case child comes into contact. 2. Child #2 and #3 do not have immunization records. 3. Child #3 does not have the name and address for parent's employment. 4. Child #2 does not have complete information for emergency contacts.

Plan of Correction: We have been working on the files and will continue. I did not realize immunization was needed on first day.

Standard #: 22VAC40-185-270-A
Description: Based on observation, there are areas inside that are not maintained. Evidence: 1. There are carpets that have frayed edges and the binding is coming undone.

Plan of Correction: We are replacing.
