Consultation was given on the new background requirements for finger printing, a revised Sworn Statement (SDS) for new staff hired after 7/1/2017 and requesting out-of-state Central Registry checks for staff who have lived outside of Virginia within the last five years. The model SDS form is online at VA DSS as well as a link to the Adam Walsh site to assist with out-of-state central registry forms. VA DSS will be paying for initial child care finger print checks until 9/30/2018. Each facility must establish a point of contact with VA DSS. All questions regarding facility codes should be directed to OBI at 804-726-7884. Eligibility letters will be sent out by e-mail and staff may not be hired until the completed letter is received. Please read, review, and respond to all correspondences regarding the fingerprint roll out. **Recent tree removal has necessitated the need for a shade structure on the playground.
An unannounced monitoring inspection was conducted today from 11:45am-1:40pm. There were 72 children present in the center today. The physical plant, staff records, children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed eating a lunch of spaghetti, salad, mixed fruit, and milk. Staff were observed engaging with children throughout lunch and offering help when needed. Smooth and calm transitions were observed when classes moved in groups to the rest rooms, to classrooms, and to the cafeteria. The inspector observed one child receive medication and viewed the daily log for ongoing medication. Since the previous visit the center has shifted record keeping to a digital platform and record keeping errors have been reduced as a result. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-70-A Description: Based on record review, not all staff records are complete. Evidence: Staff 2's record did not contain a date of hire, documentation of 2 reference checks, emergency contact information, sworn statement or affirmation, or a national criminal record check. Plan of Correction: The employee has not been on the premises due to illness, we will collect them missing information when she returns.
Standard #: 22VAC40-185-280-B Description: Based on observation, not all hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: A canister of aerosol disinfectant and air freshener were observed unlocked next to the changing table in room 111. Plan of Correction: We will move the spray bottles.
Standard #: 22VAC40-191-60-B Description: Based on record review, one employee was provided employment without a completed sworn statement or affirmation. Evidence: Staff 2's record did not contain a sworn statement or affirmation. Plan of Correction: We will have the employee complete the form prior to returning to work.
Standard #: 63.2-1720.1-B-2 Description: Based on record review, not all staff have submitted to and received the results of a national fingerprint background check. Evidence: Staff 2 does not have an eligibility letter on file as a result of an acceptable national fingerprint background check. Plan of Correction: We will not let the employee return until the fingerprint results have been received by the center.
