63.2 Facilities and Programs..

Technical Assistance:
As discussed, with the summer season approaching please monitor the status of your playground equipment, particularly the plastic playhouse. Heat tends to cause plastic to warp.

Comments:
An unannounced monitoring inspection was conducted this date. A total of 24 children with 4 staff on site. Observation of both classrooms was made. In the younger aged class, children were observed sitting in chair in a circle while listening to staff read a book to them. An audio book was also played about the "Little Red Hen" which encouraged team work. Sanitizing of the table tops was observed after playdoh time. Children had just completed a project which involved painting coffee filters with tissue paper to make shapes of the sun. The older class located upstairs were busy completing a Mother's Day project with clay. Small groups were observed either playing on the floor, the sensory table or with toy microscopes. Children were very excited about the butterfly habitat located in the class. No medications on site at this time. MAT, CPR and First Aid and Daily Health Observation training was verified. All staff files are reviewed at the administrative office on an annual basis. A total of 5 children's records were reviewed. Thank you for your cooperation today. Description: Based on child record review, one out of 5 child records reviewed did not have documentation of immunizations. Evidence: Child #4's record did not have any documentation of immunizations maintained in the record.

Plan of Correction: We will audit all records for the upcoming fall school year to ensure we have this documentation for each child.

Standard #: 22VAC40-185-140-A
Description: Based on child record review, one out of five child records reviewed did not have documentation of a physical exam. Evidence: Child #4's record did not have a copy of a physical exam maintained in the record.

Plan of Correction: We will conduct an audit of all child records to ensure all documentation is maintained for the upcoming fall school year.
