Comments:
The licensing inspector conducted an unannounced complaint inspection in response to a complaints that were received by the licensing department on March 30, 2018 and April 5, 2018 regarding allegations concerning staff supervision, ratio and forbidden acions. The inspection was initiated and completed on April 12, 2018 from 8:15am to 10:15am. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation supports part allegations; therefore, the complaint is determined to be valid. In addition to the original complaint, there were also violations found during the inspection. See the violation notice for more details. Description: Based on observation on 04/23/2018, the center did not ensure that for preschool children or younger all electrical outlets shall have protective covers. EVIDENCE: In the two year old classroom there was an uncovered electrical outlet located above the cubbies. The outlet was out of the reach of the children.

Plan of Correction: work order placed to fix/replace outlets, and/or outlet covers.

Description: Based on observation on 04/12/2018, the center did not ensure that in the school age class the staff to child ratio of one staff member for every 18 children was met. EVIDENCE: 1. The staff to child ratio in the school age classroom was one staff to 22 children. 2. The youngest child in this class were of the age of eligibility to attend public school.

Plan of Correction: Scheduled staff to arrive earlier to ensure ratio is met.

Description: Based on observation on 04/12/2018, the center did not ensure that children's hands were washed with soap and running water or disposable wipes before eating meals or snacks. EVIDENCE: In the two year old room five of the nine children did not wash their hands before eating their morning snack.

Plan of Correction: Staff training on handwashing was reviewed with all staff.

Description: Based on observation on 04/12/2018, the center did not ensure that tables are sanitized before each use for feeding. EVIDENCE: 1. In the two year old room, the children were observed playing with toy cars and other toys at the tables that they eat their morning snack on. 2. The same tables were not sanitized before the children ate there snack on them.

Plan of Correction: Staff training on cleaning and sanitation was reviewed with all staff.
