Technical Assistance:
Consultation was given on the new background requirements for finger printing, a revised Sworn Statement (SDS) for new staff hired after 7/1/2017 and requesting out-of-state Central Registry checks for staff who have lived outside of Virginia within the last five years. The model SDS form is online at VA DSS as well as a link to the Adam Walsh site to assist with out-of-state central registry forms. VA DSS will be paying for initial child care finger print checks until 9/30/2018. Each facility must establish a point of contact with VA DSS. All questions regarding facility codes should be directed to OBI at 804-726-7884. Eligibility letters will be sent out by e-mail and staff may not be hired until the completed letter is received. Please read, review, and respond to all correspondences regarding the fingerprint roll out. Discussed withh center management hand washing, children's records.

An unannounced monitoring inspection was conducted from 10:30 am-2:00pm. 12 children were in care with 2 staff members present. Children were observed during snack time (snackincluded fruit bars, yogurt and water), engaged in circle time learninf about days of the week, counting numbers, and were also observed reading books. A fire drill was conducted during the inspection. A selection of staff and children records and emergency drill records were reviewed. Areas of non-compliance are identified in the violation notice. Description: Based on record review, center did not maintain complete information in children's records. Evidence: The dates for the first day of attendance for child # 1, 2, 3, 4, and 5 were not listed in their records.

Plan of Correction: We will have the dates completed by Friday.

Standard #: 22VAC40-185-500-A
Description: Based on observation, children's hands were not washed after eating snack. Evidence: 1. The children ate morning snack. 2. Hand washing was not done after the children finished eating their snack. The children proceeded to the next activity.

Plan of Correction: We will make it part of protocol to make sure hands are being washed before and after meals and snacks.

Standard #: 63.2-1720.1-B-2
Description: Based on record review, center did not obtain fingerprinting results for a new employee prior to employment. Evidence: 1. Staff # 1 was hired on 1/29/18. 2. Staff # 1's record indicated that the center had obtained fingerprinting results from Virginia State Police but not from the state approved vendor, Field Print. 3. The requirement for Field Print fingerprinting went into effect 1/16/18.

Plan of Correction: We will obtain the field print results.

Description: Based on record review, center did not obtain out of state central registry search results for employees that have resided outside of Virginia in the past five years. Evidence: 1. Staff # 1 was hired on 1/29/18. 2. Center has not obtained out of state central registry searches for two states where Staff # 1 had resided in the past five years.

Plan of Correction: We will obtain the out of state central registry for the staff member.
