22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted today from 10:45am-1:30pm. There were 67 children (infants-5yrs.) supervised by 10 direct care staff. The physical plant, 5 staff records, 6 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in gymnastics classes, transitioning to outdoor play, and bottle feedings. Diapering and hand-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-240-D-5 Description: Based on observation, staff records did not contain documentation of obtaining daily health observation training within the last three years. Evidence: 5 of 5 staff records did not document daily health observation training. Plan of Correction: A link to the online training was sent to our opening and closing lead during inspection. Training will be completed by the end of the day.
Standard #: 63.2-1725-A Description: Based on record review, 1 of 5 staff records did not contain appropriate documentation of a completed sworn disclosure statement prior to date of hire. Evidence: Staff C's record did not contain documentation of the most recent sworn disclosure statement to indicate if the employee has lived outside of Virginia within the past 5 years. Plan of Correction: We will update this right away before the end of the day. Staff C signed the outdated sworn statement form from 2017. We will have Staff C sign the 2018 version of the sworn statement once she arrives to work this afternoon. The completed fingerprints and the VA background check are up to date and were in the staff's file during inspection.
