A Subsidy Health and Safety inspection was conducted on 5/7/18 to review supplemental health and safety requirements. An unannounced monitoring inspection was completed on 5/7/18. There were 32 children present. Five children's records and two records from the previous inspection were reviewed. Five staff records and one from the previous inspection were reviewed. Three medications were observed. The inspector arrived at the center at 8:30 am and departed at 2:45 pm. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: VENDSUB-000-012-B Description: Based on record review and interview, the center failed to ensure that within the first 90 days of employment staff had completed the required Virginia Preservice Training for Child Care Staff. Evidence: 1. Staff #5 with a hire date of 11/6/17 had not completed the VA Preservice training. 2. Staff #5 confirmed that the training had not been completed. Plan of Correction: The staff person will complete the training.
