Comments:
The licensing inspector conducted an unannounced monitoring inspection Monday, May 7, 2018 from approximately 1:25PM to 5:00PM. The following ratios were observed with a total of 127 children present during the inspection:
Satellite Infants - 2 children with 1 staff
Young Infants - 7 children with 2 staff
Middle Age Infants - 4 children with 1 staff
Older Infants (Red Room) - 7 children with 2 staff
Transitional Toddlers (Red Room) - 9 children with 2 staff
Toddlers (Orange Room) - 14 children with 3 staff
Twos (Yellow Room) - 16 children with 2 staff
Threes (Green Room) - 17 children with 2 staff
Threes (Blue Room) - 16 children with 2 staff
Fours (Purple Room) - 17 children with 2 staff
Fours & Fives (Gold Room) - 17 children with 2 staff
Get Well - 1 child with 1 staff

The children and staff were engaged in a variety of activities when the licensing inspector arrived. Infants were observed playing on the floor with their teachers, playing outside, eating snack and sleeping in their cribs. The Toddlers were observed looking at books, eating snack, having their diapers changed and playing outside. The Twos and Threes classrooms were observed eating snack, resting at naptime, taking bathroom breaks and washing hands and playing outside. The Fours and Fives were observed playing outside, eating snack, learning about the United States and reviewing sign language. Snack consisted of animal crackers and milk. All areas of the center including classrooms, hallways, kitchen, playgrounds, bathrooms and playgrounds were inspected. The center was equipped with toys and supplies and items were available to the children.

The following information was reviewed: injury reports, daily sheets, daily attendance, infant feeding schedules, practiced emergency drill logs, parent agreements, staff training information and liability insurance. The center's first aid kit and emergency supplies were inspected. Medication is administered, and medication and authorization forms were reviewed.

Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevented access by children. Evidence: During the inspection, the licensing inspector observed two unlocked chemicals that were located in an unlocked cabinet in the Green Room. The containers of chemicals were labeled "Warning: Keep out of the reach of children". A member of management moved the chemicals to a locked cabinet during the inspection.

Plan of Correction: Cabinet was locked during inspection and a staff memo was distributed to remind staff to let someone know when they need a lock. Safety equipment is always available.

Standard #: 22VAC40-191-60-B
Description: Based on a review of 10 staff records, the center did not obtain a completed sworn statement prior to two staff members beginning employment. Evidence: During the inspection, the licensing inspector observed the records for Staff #6 and Staff #9 contained sworn statements dated 03/01/18. The documentation in the records indicated Staff #6 and Staff #9 completed orientation on 02/26/18. Staff are required to complete a sworn statement prior to beginning employment.

Plan of Correction: Pre screening includes fingerprinting and full background checks to be completed before orientation. Sworn Disclosures were completed before new hires were with children. The sworn disclosure will now be the first form completed with orientation.
