22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection took place between the hours of 9:30am and 12:00pm. There were a total of 74 children with 12 staff. A total of 5 children?s files, 5 staff files, medications and emergency preparedness procedures. were reviewed. The children were engaged in the following: playing outside, eating breakfast, playing with sand and other organized activities. Areas of non-compliance are identified in this report. Standard #: 22VAC40-185-60-A
Description: Based on review of five children?s records, the provider did not maintain complete children?s records. Child #1,2,3,&4 do not have documentation of child updates and confirmation of up-to-date information. Last updates were February 2017.

Plan of Correction: I will contact the parents and document updates for all children enrolled.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the provider failed to cover all unused electrical outlets accessible to children under five years of age with child-resistant protective covers. Evidence: Electrical outlets did not have protective covers in the Intermediates and Pre K 2 classrooms.

Plan of Correction: Corrected day of inspection.

Standard #: 22VAC40-185-320-B
Description: Each rest room area must be provided with paper towels and have working toilets. Evidence: 1. No paper towels were available in the Intermediates, the Pre K 1 and the Pre K 2 classrooms. 2. One toilet is not in working order in the Pre K 1.

Plan of Correction: Paper towels will be distributed to each classroom. I'm going to fix the toilet today.

Standard #: 22VAC40-185-500-B
Description: The diapering surface shall be used only for diapering or cleaning children. Evidence: Observation was made that other items are being stored on the changing table in both Toddler and Infant rooms, ie: deli paper and food storage bags.

Plan of Correction: I will staff with staff today and remove all items.

Standard #: 22VAC40-185-510-A
Description: Based on review of nine medications, the center did not have written authorization to administer medication to a child in care. Evidence: Child #7 did not have medication authorization on the correct form.

Plan of Correction: I contacted mom to have the doctor complete the correct form.

Standard #: 22VAC40-185-510-C
Description: Based on observation of medications, the center did not ensure there are no out of date medications. Evidence: Child #5, (medication expires 9/16) and child #6, (medication expires 4/18) had medication on site.

Plan of Correction: I will return medication to mom today. Medications will be reviewed monthly to ensure no expired medications are on site.
