Thank you for your time and assistance with the unannounced monitoring inspection conducted today, May 10, 2018. The on-site inspection was from 10:10a.m. to 1:30p.m. On this date, there were 17 children present and in the care of two staff members. There were a total of 20 children enrolled in the preschool program. There were five children's records reviewed on this date. Staff records were reviewed on 09/12/2017. Two staff members and three children were interviewed. There were no medications on-site or being administered. Lunch was provided by the school and consisted of beef & cheese tacos, black beans, fresh oranges, and milk. The exceptions to compliance were noted on the violation notice and the risk ratings were listed on the supplemental page. The theme for the week was Dinosaurs. Some of the items observed and reviewed were: required posted information, documentation of evacuation and shelter-in-place drills, emergency supplies, first aid kit, injury reports, transitions, center time, bathroom and hand washing procedures, outdoor play, story time, lunch, and nap time. Standard #: 22VAC40-185-280-B Description: Based on observation and discussion with staff, the center failed to keep hazardous substances locked. Evidence: 1. In one unlocked cabinet in the classroom there were three containers of Licefree Spray, a container of Germ X Hand Sanitizer, and a spray can of Rid Lice Spray. The cabinet was accessible to the children. 2. Staff Member A reported that the lock on the cabinet had broken and she had requested through the Family Advocate a new lock, but it has not been replaced. Plan of Correction: During the inspection, the staff took all of the hazardous items in the cabinet and put them in a locked box. Staff will again request a new lock for the cabinet.
Standard #: 22VAC40-185-540-B Description: Based on observation and confirmation from staff, the center failed to keep the first aid kit inaccessible to the children. Evidence: 1. During lunch in the cafeteria, the first aid kit was not locked and was sitting on a lunch table accessible to the children. 2. During outdoor play, the first aid kit was hanging on the fence, was not locked, and was accessible to the children. 3. Staff Member A reported that the lock on the first aid had broken and she had requested through the Family Advocate a new lock, but it had not been replaced. Plan of Correction: Staff will again ask administration for a new lock and will follow-up to ensure it is obtained.
