Comments:
An unannounced monitoring inspection was conducted from 10am - 12:45pm. There were 6 children in care ages 2 - 4 years old with three staff. Children and staff records were reviewed. Children were observed in teacher and self-directed activities. Documentation of emergency evacuations, medication and transportation were reviewed. Violations regarding physical plant, supervision, nutrition and food service, sanitation transportation and medication were identified and appear on the violation notice.

Description: Based on observation the center failed to ensure that areas and equipment are maintained in safe condition. Evidence: 1 - In the preschool room there is a cracked plastic adult chair. a. The crack pinched the skin of an adult as she was standing up after sitting in the chair. 2 - Also in the preschool room there is broken blind slat within reach of children. a. The broken slat has jagged edge.

Description: Based on observation and interview the center failed to ensure that hazardous substances are kept in a locked place. Evidence: 1 - There is bottle of rubbing alcohol on the teachers desk in the preschool room, an unlocked location. 2 - Staff 2 confirmed that the alcohol is stored on the desk within reach of children.

Description: Based on observation and interview the center failed to ensure the children under 10 years of age are always within actual sight and sound supervision. Evidence: 1 - Child 1 was observed as she walked alone from the bathroom into the preschool classroom. a. The bathroom is located across hall and around a corner from the classroom. b. Staff 2 could hear child 1 while she was in the rest room. c. Staff 2 stated that she did not know about sight and sound supervision.

Description: Based on observation and interview the center failed to ensure that the most recent USDA requirements. Evidence: 1 - The morning snack served to children in care today was oranges. There was not any other food or drink offered. USDA regulations require two components be served at snack time. 2 - Staff 1 confirmed that oranges was the only item served with morning snack. 3 - The lunch served to children in care was peanut butter and jelly sandwiches, 1/4 orange, celery and juice. a. The center cook indicated that she did not if the required amount of peanut butter to equal the protein requirement was served to each child. b. The center cook indicated that she did not measure the amount of fruit and vegetables served to each child. c. USDA requires that milk be served with each meal. Children were served juice. No milk was offered.

Description: Based on observation the center failed to sanitize tables before meals and snacks. Evidence: 1 - The table used for lunch was not sanitized before lunch was served. a. The table was sprayed with sanitizer and immediately wiped dry. The surface was not allowed to air dry.
