Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 7/7/2025 from 10:45am -1:45pm.
The Licensing Inspector met with the Director of Family Engagement for the entrance conference and preliminary finding meeting.
Number of staff records reviewed (3)
This office is a location for child-placing activities to occur at this location, but no active family records are currently housed at this Norfolk office location.
There are no birth parent/child or provider home case records maintained at this location, although the office is available to conduct any needed child placing activities. Human Resource department and personnel records are located at this location. The agency has hired a new staff since last inspection. The physical office was inspected and is in compliance with the standard 22VAC40-131-50. The agency's license and results of the most recent licensing inspection were posted and in compliance with 22VAC40-131-50.
An exit meeting was conducted with the Executive Director on 7/9/2025.
The evidence gathered during the inspection determined (2) violations with applicable standard(s) or law. Standard #: 22VAC40-131-110 Description: Violation:
Based on record review for Personnel record (P1), it was determined that the agency failed to indicate the date information was received by licensee on several required documents, as required by regulation.
Findings:
1. Personnel (1) was hired by licensee, 11/2024. Upon review of the personnel file, multiple documents were found to be missing the date of receipt by the licensee. Examples of such documents included the DMV driving record, auto insurance documentation, education transcripts and background check results.
2. The preliminary findings were reviewed with AR (2). AR2 was provided with the opportunity to review the personnel record and the associated documentation. AR(2) acknowledged the findings as reported by the licensing representative following reviews of the record.
3. The findings were discussed during the exit meeting with Agency Representative (AR1) who acknowledged the inspection findings.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 22VAC40-191-40-D-1-b Description: Violation:
Based on the record review for Personnel record (3), it was determined that the agency failed to obtain the results of the criminal history check within 30 days of employment with the adoption program Personnel #3 was hired 4/1/25 for the license adoption program.
Findings:
1. Personnel (3) were hired on 4/1/25 to work with the licensed adoption program.
2. The record for P (3) documented the results of the criminal history check (Virginia State Police name search) with a date of 5/14/25 which exceeds the 30-day window from the date of hire for the adoption program.
3. The preliminary findings were reviewed with AR (2). AR (2) was provided with the opportunity to review the personnel record and the associated documentation following the review. AR (2) acknowledged the findings as reported by the licensing representative.
4. The findings were discussed during the exit meeting with Agency Representative (AR1) who acknowledged the inspection findings.
