Type of inspection: Monitoring Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 7/21/2025 from 10:45- 1:00 p.m. The Licensing Inspector met with the administrative assistant for the entrance conference. Number of home provider records (9) Number of home provider records reviewed (1) Number of birth parent/child record (3) Number of birth parent/child record reviewed (1) Number of staff records reviewed-(0) Staff record reviewed at this location. Records reviewed during another license location on 7/2025. The agency has not hired any new staff since that last inspection. The physical office was inspected and is in compliance with the standard 22VAC40-131-50. The agency's license and results of the most recent licensing inspection were posted near the agency's entrance. The preliminary finding meeting was held on 7/21/25 with the Adoption Specialist and an exit meeting was conducted with the Chief Executive Officer on 7/22/2025. The evidence gathered during the inspection determined (1) violations with applicable standard(s) or law. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law. If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview. Compliance with all applicable regulations and laws shall be maintained and any areas of noncompliance must be corrected. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary. Standard #:	22VAC40-131-210-A
Description:	Violation: Based on review of the records for adoptive provider homes (AH1) the licensee failed to complete all of the core competencies during pre-service training provided to the adoptive family home providers. Findings: 1) During the review of the record for Adoptive Home (AH1), it was determined that there was no documentation to show that all required core competencies were completed during the pre-service training for (AH1). The following competencies were not available at time of review and during the exit meeting. a. Reunification as a primary child welfare goal, including processing the experience of reunification. 2. The preliminary findings were discussed on 7/21/25 with Staff 1 (S1) and Staff 3 (S3). 3. The findings were reviewed during the exit meeting with S2 who acknowledged the inspection findings.
