Type of inspection: ?Monitoring? A mandated monitoring inspection was conducted on 11/5/25 from 1:00 p.m. to 4:00 p.m. An entrance meeting and preliminary findings review were conducted with the Chief Program Officer and Program Manager. An exit meeting was held on 11/5/25 with the Chief Program Officer, Program Manager. Number of youth: 8 Number of approved provider homes: 10 The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Number of children/youth records reviewed: 2 Number of provider records reviewed: 2 Number of staff records reviewed: 1 Number of interviews conducted with staff: 2 Additional Comments/Discussion: An exit interview was held on the date of inspection. LCPA has converted to electronic documentation. The evidence gathered during the inspection determined no violations with applicable standard(s) or law.
