A monitoring inspection was initiated on 10/05/2021 and concluded on 10/06/2021. The Director was contacted by telephone to initiate the inspection. The Director reported that the agency currently has 25 families approved and waiting for placement and 4 children in its custody. The inspector emailed the Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed three (3) adoptive home records, one (1) birth parent/child file, and one (1) personnel record submitted by the agency to ensure documentation was complete. An exit interview was conducted with the Director on 10/06/2021, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violation was documented on the violation notice issued to the agency.
Description: Violation: Based on a review of documents from an adoptive home file (AH-3) submitted by the agency and an interview with an agency representative (AR-1), the agency failed to obtain a signed authorization from the applicant to allow the previous child-placing agency to release information to the licensee about previous applications. Evidence: 1) In the application dated 2/22/21, AH-1 documented that they previously completed an application with another child placing agency and were on that agency's pre-waiting list. 2) There was no signed, consent to exchange information form in the file of AH-1. 3) In an exit interview on 10/06/2021, AR-1 acknowledged that there was no signed, consent to exchange information form in the file of AH-1.
