Comments

Type of inspection: Monitoring An unannounced mandated monitoring inspection was conducted. The Executive Director and Senior Director of Clinical and Support Services were notified via phone and email. The inspection was conducted onsite from 10:45am to 4:30 pm. An entrance conference was held with the Director of Clinical and Support Services, Human Resources Manager and Adoption Home Study Case Manager and these staff were available during the inspection. Record reviews were conducted electronically. The Acknowledgement of Inspection form was signed and left at the facility. The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. Number of youth in placement: 14 Number of youth records reviewed: 3 Number of interviews conducted with youth: 0 Number of staff: 29 Number of staff records reviewed: 6 Number of interviews conducted with staff: 3 Number of provider homes: 28 Number of provider home records reviewed: 3 Observations by licensing inspector: License and last inspection summary posted in facility as required. Additional Comments/Discussion: A Preliminary findings meeting was conducted. Also an exit meeting was conducted on November 10, 2025 to review the findings. Observations, interviews, and record reviews during the inspection determined there were no violations with applicable standard(s) or law.
