Type of inspection: Renewal An unannounced Renewal inspection was conducted onsite from 10:30am to 3:30pm on day 1 and 11:00 am to 1:00 pm on day 2. The entrance conference was held with the Senior Director of Clinical Support Services. The Human Resources Manager and Adoption Home Study Case Manager were accessible during the inspection. Electronic record reviews were conducted. The Acknowledgement of Inspection form was signed and left at the facility for each day of the inspection. The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. Number of children in placement: 2 Number of children’s records reviewed: 1 Number of interviews conducted with children: 0 Number of provider homes: 32 Number of provider home records reviewed: 3 Number of staff records reviewed: 17 Number of interviews conducted with staff: 1 Observations by licensing inspector: License and last inspection summary posted in facility as required. Additional Comments/Discussion: A Preliminary findings meeting was conducted. Also an exit meeting was conducted to review the findings. Observations, interviews, and record reviews during the inspection determined there were no violations with applicable standard(s) or law. The Inspection Summary will be posted to the VDSS website within five (5) business days of your receipt of the Inspection Summary.
