Type of inspection: Monitoring An unannounced inspection was conducted on day one from 11:00am to 4:00pm and day two from 10:15am to 12:00pm. The notification was made to the Program Director and Adoption Supervisor. The entrance conference was held with the Adoption Supervisor. The Adoption Supervisor remained onsite for the inspection. Records were reviewed electronically. The Acknowledgement of Inspection form was signed and left at the facility. The preliminary findings meeting was conducted onsite. The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. Number of children in care: 7 Number of child records reviewed: 1 Number of interviews conducted with children: 0 Number of provider homes: 34 Number of provider home records reviewed: 4 Number of Staff: 8 Number of staff records reviewed: 3 Number of interviews conducted with staff: 1 Observations by licensing inspector: License posted as required A final exit meeting will be conducted Additional Comments/Discussion: The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law. If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview. Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected. Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing Administrator at the regional licensing office that serves your geographical area. Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice. Description: Based on review of the record for Provider Home one (PH1), the Licensee failed to consider all information received about an applicant, prior to making a decision. Findings: 1) The record for PH1 contained driving records for two adults that will be responsible for transporting children. 2) The driving records for both adults were dated 10/20/2025. 3) The driving record report for one of the adults showed that the adult incurred a moving violation. 4) There was no supporting documentation in the record, or indication in the home study verifying that the information contained in the driving report was assessed, evaluated or summarized prior to making the approval decision. 5) The findings were reviewed at the Preliminary Findings meeting and S2 acknowledged the findings.

Description: Based on review of the record for provider home five (PH5) the licensee failed to obtain a central registry check for a person 14 years of age and older, or turning 14. Findings: 1) The record for (PH5) contained current and approved background checks for the foster parents and one person living in the home over the age of 14. 2) There is an additional person living in the home aged 14 or over. 3) There was no supporting documentation in the record, or indication that a central registry check was completed, reviewed and approved for the second person 14 years of age and older, or turning 14. 4) The findings were reviewed at the Preliminary Findings meeting and S2 acknowledged the findings.
