Comments

An unannounced, monitoring inspection was conducted on January 13, 2022 from 11:00 am to 2:00 pm. Siena Adoption Services continues to provide home studies for adoptive families and post placement supervision services. The agency is currently working with four approved providers. During this inspection, the new office space was observed for compliance with regulations regarding office space and conditions and three adoptive home records were reviewed. No personnel records were reviewed as no new staff were hired since the most recent inspection. There was one citation for a violation of the Standards for Child-Placing Agencies. It was a key, health and safety standard. The Case Worker was available for the inspection and present for the exit interview at the end of the inspection on 1/13/22. Upon receipt of the violation notice, the licensee must develop a plan of correction for each violation. The plan of correction must include the following: the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which non-compliance will be corrected. The licensee has ten (10) calendar days from receipt of the inspection documentation to complete the section entitled "Plan of Correction", sign each page of the Plan of Correction and return it to the Licensing Office. The licensee retains a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the VDSS web site within five (5) days, regardless of whether or not the Plan of Correction has been completed.

Standard: 22VAC40-131-(5)-180-J-2

Description: Violation: Based on a review of an adoptive family file (AH-2) and an interview with an agency representative (AR-1), the agency failed to document that the applicants have in-force vehicle insurance as required. Evidence: 1) The adoptive family file of AH-2 did not contain documentation of in-force vehicle liability insurance. 2. AR-1 acknowledged that documentation of in-force vehicle liability insurance was not contained in the family's file.
