A renewal inspection was conducted on 3/1/2022 from 9:45 a.m. to 1:30 p.m. An entrance conference was conducted with the State Treatment Foster Care Director and an Assistant. The agency reported a current census of 11 children in placement and 13 approved homes. The inspector reviewed two (2) foster child, three (3) personnel, and two (2) provider home records. During the license period 50% of personnel records were reviewed. Background checks for Board Officers were reviewed during a renewal inspection of another office location in February 2022. An exit interview was conducted with the Co-Executive Director, State Treatment Foster Care Director, Lead Clinical Specialist, and an Administrative Assistant on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. An acknowledgement of inspection was signed and left with the agency at the end of the inspection and inspection documents were sent for signature by email for signature. The information gathered during the inspection determined there was no non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the facility.
