A monitoring inspection was initiated on November 8, 2021 and concluded on November 17, 2021. The Executive Director was contacted by telephone to initiate the inspection. The Executive Director reported there were 24 approved foster homes and 28 children in foster care. The inspector emailed the Executive Director a list of items required to complete the inspection. The inspector reviewed three personnel records, three foster home records, and two children’s records submitted by the agency to ensure documentation was complete. Exit meeting was held with the Executive Director on November 17, 2021. Information gathered during the inspection determined non-compliance with applicable standards or law, and a violation was documented on the violation notice issued to the agency.
Description: Violation: Based on review of the records for foster homes FH1 and FH2 and interview with agency representatives AR1 and AR2, the agency failed to maintain documentation in the provider files, as required by this standard. Findings: FH1: 1. The 05/27/2021 and 08/16/2021 90 day monitoring visits were not placed in the record for foster home FH1 within the 30 days required by this regulation. 2. AR2 acknowledged that the documentation for the 90 day monitoring visits were not placed in the record within the timeframe required by this regulation. FH2: 1. The 06/29/2021 and 09/28/2021 90 day monitoring visits were not placed in the record for foster home FH2 within the 30 days required by this regulation. 2. AR2 acknowledged that the documentation for the 90 day monitoring visits were not placed in the record within the timeframe required by this regulation.
Director will retrain foster parent coordinators on importance of filing the 90-day monitoring reports in eR in a timely manner
