An unannounced monitoring inspection was conducted at the Roanoke agency on March 21, 2022 from 12:25 p.m. to 3:57 p.m., on March 22, 2022 from 9:48 a.m. to 3:40 p.m. and at the Fredericksburg agency to review personnel records on March 23, 2022 from 9:30 a.m. to 11:32 a.m. The agency reports forty three (43) children in foster care. One staff was hired since the last inspection. During this inspection, the inspector reviewed one (1) personnel record, four (4) children’s records, and four (4) foster home records. Preliminary inspection findings were reviewed at the conclusion of the inspection with agency representative AR1. An acknowledgement form was signed. Upon receipt of the violation notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.
Description: Violation: Based on a review of the foster home record for FH1 and an interview with agency representative AR1, the agency failed to include the driving record obtained from the Department of Motor Vehicles for the applicant. Findings: 1. FH1 was approved on 03/01/2022. The record contained in the file for the applicant, Foster Parent A, was a vehicle record but not the driving record. 2. AR1 acknowledged that the record contained in the file for Foster Parent A was not the driving record.
Director will train FPC to look for driving record rather than motor vehicle record. FPC will work with foster parent to obtain the correct report. QA Specialist will be trained to look for correct DMV report in FP charts
