An unannounced renewal inspection was completed by the Licensing Specialist on 3/17/22 from 10:12 a.m. to 3:12 p.m. at the corporate office in Richmond, where the staff records are maintained, for the purpose of reviewing staff records for this agency. Also, an unannounced monitoring inspection was conducted by the Licensing Specialist on 3/18/22 from 12:09 p.m. to 3:45 p.m. at the agency. The current census is five (5) youth. The following is a listing of the activities for this inspection: One youth record was reviewed. Two staff records were reviewed. On 3/17/22, the Manager of Compliance and Development was available and accessible during the inspection. The Acknowledgment of Inspection form was signed and left at the agency. On 3/18/22, the Senior Life Skills Coach Trainee and the Admissions Coordinator were available and accessible during the inspection. The Assistant Director was accessible by phone and interviewed in order to provide clarification about a particular document. The Acknowledgment of Inspection form was signed and left at the agency. The licensee should retain a copy of this document to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services’ website.
