Type of inspection: Monitoring Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 9/8/22 from 12:21 PM – 3:00 PM and 9/16/22 from 10:10 AM – 3:41 PM The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. Number of youth in care: 3 The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Number of children’s records reviewed: 1 Number of youth records reviewed: 1 Number of interviews conducted with staff: 1 Additional Comments/Discussion: An entrance conference was held at the agency on 9/8/22. Also, on the same date, a youth’s record was reviewed at the agency. The Regional Supervisor was available and accessible during the inspection. The Executive Director was interviewed by phone on the same date in order to provide clarification about several documents reviewed. Staff records were reviewed at the corporate office in Richmond on 9/16/22. The HR Business Partner and the HR Generalist were available and accessible during the inspection. An exit meeting was conducted on 9/29/22 to review the inspection findings. The evidence gathered during the inspection determined no violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary.
