Type of inspection: Complaint Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 2/10/23 from 9:40 AM – 1:16 PM 2/21/23 from 11:44 AM – 6:13 PM 2/22/23 from 12:14 PM – 5:52 PM The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. Complaints were received by VDSS Division of Licensing on 1/31/23 regarding allegations in the area(s) of: Independent Living Arrangements. Number of youth in care: 3 Number of youth records reviewed: 1 Number of interviews conducted with staff: 5 Additional Comments/Discussion: On 2/10/23, a staff record was reviewed at the corporate office in Richmond. The HR Business Partner was available and accessible during this inspection. The HR Business Partner was interviewed to obtain clarification about a few documents. On 2/21/23, an entrance conference was held at the agency with the Executive Director. While at the agency, the following was reviewed: applicable sections of a youth’s record, the medication log, and the log book. The Executive Director and the Evening Support Specialist were available and accessible during the inspection on 2/21/23. The Regional Supervisor and the Evening Support Specialist were available and accessible during the inspection on 2/22/23. The following staff were interviewed: Executive Director on 2/21/23, Life Skills Coach on 2/21/23, Regional Supervisor on 2/22/23, and Evening Support Specialist on 2/21/23 and 2/22/23. The following staff were interviewed to provide clarification about several documents reviewed in regard to the youth: Evening Support Specialist on 2/22/23 and Executive Director on 3/9/23. An exit meeting was conducted on 5/5/23 to review the inspection findings. The evidence gathered during the investigation did not support the allegations of non-compliance with standard(s) or law.
