Type of inspection: Complaint Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 9/5/23 from 10:50 AM – 6:12 PM – This inspection was conducted simultaneously with another complaint inspection. The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. Complaints were received by VDSS Division of Licensing on 6/20/23 and 6/22/23 regarding allegations in the area(s) of: Independent Living Arrangements. Number of youth in care: 5 Number of staff records reviewed: 1 Number of interviews conducted with staff: 3 Additional Comments/Discussion: On 9/5/23, an entrance conference was held at the agency with the Executive Director. The Executive Director was available and accessible during this inspection. While at the agency, the following were reviewed: the log book and a personnel file. Two youth were interviewed during this inspection. The following staff were interviewed at the agency: Executive Director, Senior Life Skills Coach, and Evening Support Specialist. A preliminary findings meeting was held at the agency. The HR Business Partner was interviewed by video call to obtain clarification about a personnel document. An exit meeting was conducted on 10/5/23 to review the inspection findings. The evidence gathered during the investigation did not support the allegations of non-compliance with standard(s) or law. The inspection summary will be posted to the VDSS website within 5 business days of your receipt of the inspection summary. The department's inspection findings are subject to public disclosure.
