Type of inspection: Monitoring Date(s) of inspection: An unannounced onsite inspection was held on 10/24/2024 from 11:15 a.m. to 2:20 p.m. A preliminary meeting was held on the same day as the onsite inspection. The Acknowledgement of Inspection form was left at the facility. Number of IL Youth in care: 4 The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Number of ILA youth records reviewed: 2 (includes 1 discharge) Number of staff records reviewed: 3 Number of interviews conducted with staff: 1 Observations by licensing inspector: Documentation needed for review was readily available/accessible. Exit meeting held on 10/28/2024. Additional Comments/Discussion: Regional Supervisor was onsite during entrance and available throughout the inspection to answer questions and providing documentation needed for the inspection. ILA Director, HR Business Partner and ILA Assistant Director were available remotely/email correspondences. The evidence gathered during the inspection determined no violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary.
