Type of inspection: Complaint Date(s) of inspection: An announced onsite inspection was held on 05/19/2025 from 10:50 am to 2:20 pm and again on 05/28/2025 from 10:45 am to 4: pm. Ongoing review dates occurred on 06/30/2025, 07/01/2025 and 07/02/2025. The Acknowledgement of Inspection form was provided. Preliminary reviews on the date of onsite inspections. Number of ILA Youth in care: 6 The licensing inspector completed a prior tour of the physical plant that included the office setting and conditions. Number of ILA youth records reviewed: 8 (total includes 2 discharges) Number of staff records reviewed: 0 Number of interviews conducted with staff: 5 Observations by licensing inspector: Documentation needed for review was readily available/accessible. Exit meeting was held 07/21/2025 Additional Comments/Discussion: ILA Director, and ILA Assistant Director were available. The evidence gathered during the investigation did not support the (allegation(s) of non-compliance with standard(s) or law.
