Technical Assistance:
Recommendation was made for new key staff to attend Assisted Living Facility Phase II training.

Comments:
An unannounced monitoring inspection was conducted on 5/23/19 by 2 licensing inspectors from approximately 10:40 a.m. - 3:00 p.m. The current resident census reported is 73. During the inspection, a tour of the facility was conducted, a sample of 10 resident and 5 staff records were reviewed, facility documentation was reviewed, meal and activity calendars were reviewed, lunch meal was observed, water temperature was checked, medication pass was observed, medication administration records and physician orders were reviewed, interview of staff and residents were conducted and no issues or concerns were reported. All new personnel records were reviewed since the last inspection for criminal history record reports and all were in compliance. An exit meeting was held. Violations were cited during this inspection. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return to the Inspector within 10 days. You will need to specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. Your plan of correction must contain: 1) steps to correct the non-compliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Standard #: 22VAC40-73-1100-A
Description: Based on the review of resident records, the facility failed to obtain written approval of the resident, guardian, other legal representative or relative who is willing and able to take responsibility to act as resident's representative prior to placing a resident with a serious cognitive impairment due to a primary psychiatric diagnosis of dementia in a safe, secure environment. Evidence: There was no written approval available for review in resident #10 record to support placement in safe, secure unit.

Plan of Correction: A written approval for placement in secure unit for resident #10 was signed and received by resident?s Power of Attorney and filed in resident?s record on 5/28/2019. The Executive Director and/or designee will ensure prior to placing a resident with a serious cognitive impairment due to a primary psychiatric diagnosis of dementia in their memory care unit will obtain a written approval for placement in secure unit by the appropriate legal representative.

Standard #: 22VAC40-73-260-A
Description: Based on review of staff records and facility documentation, the facility failed to ensure that all new direct care staff who do not have current certification in first aid, receive certification in first aid within 60 days of employment . Evidence: 1) Staff #3 hire date noted in record as 4/5/19; no proof of first aid certification available for review, at time of inspection. 2) Staff #4 hire date noted in record as 3/15/19; no proof of first aid certification available for review, at time of inspection. 3) Staff #5 hire date noted in record as 12/3/18; no proof of first aid certification available for review, at time of inspection.

Plan of Correction: A First Aid and CPR class was held on 6/5/2019. Staff #3, #4 and #5 all have a current first aid certification in the their personnel record. The Business Office Manager and/or designee will ensure all newly hired team members have proof of certification in first aid within the first 60 days of employment.
