Technical Assistance:
Consultation provided on uncovered outlets, keeping hazardous substances locked, making sure all bathrooms have disposable paper towels, labeling the contents of your spray bottles that contained cleaning agents
Comments:
An initial inspection was conducted today with the director and assistant director. The space inside and outside were inspected as the space had previously been measured and supports the requested capacity. The classrooms were appropriately furnished and had ample supplies on site or ordered for the requested age range. The cribs in both Infant A and B classrooms were manufactured in 2012 or 2021. Please provider the revised staff and parents handbooks as well as the written policies/procedures outlined on the violation notice to Licensing by 7/19/22. The areas of non compliance are outlined on the violation notice.
Based on interview, it was determined that the center had not developed written procedures for injury prevention.

We will provide the missing policy and procedures.

Description:
Based on interview, it was determined that there were no developed written procedures for prevention of abusive head trauma, coping with crying babies, safe sleep practices and sudden infant death syndrome awareness.

Description:
Based on observation, it was determined that the center was not maintained in a a safe or operable condition. Evidence: The Licensing Inspector observed on the gates and fence surrounding the playground were protruding bolts more than two threads which presented an impalement hazard.
The Licensing Inspector observed that there were four safety belts missing from the infant buggy.
Plan of Correction:
We will cut the bolts to ensure they no longer present a hazard and the safety straps for the buggy has been ordered.

Description:
Based on interview, it was determined that the center failed to develop written policies and procedures that describes how the center will ensure that each group of children received care by consistent staff or team of staff members.

Description:
Based on observation, it was determined that 3 crib mattresses were torn, therefore were not covered with a waterproof material which can be cleaned and sanitized.

Description:
Based on observation and interview, it was determined that the infant feeding table seats were missing the protective belt in the two infant rooms. Evidence: There was one missing protective belts from the feeding table seats from both Infant A and Infant B rooms.
Plan of Correction:
The safety belts have been ordered for the feeding table.
