Comments:
An unannounced, mandated, monitoring inspection was completed May 8, 2019 from 8:30 am to 1:45 p.m. The agency reported 15 children in placement and 22 approved provider homes at the time of inspection. During this inspection, three foster home records, three foster child records were reviewed. No new staff were hired since the last inspection. The program administrator was available for the inspection and for the exit interview at the end of the day. There were two citations for violations of the Standards for Child Placing Agencies. Both were key health and safety violations. Please see the violation notice on the Department's web site for violations of the Standards. Upon receipt of the violation notice, the licensee must develop a plan of correction for each violation. The plan of correction must include the following: the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which non-compliance will be corrected. The licensee has ten (10) calendar days from receipt of the inspection documentation to complete the section entitled "Plan of Correction", sign each page of the Plan of Correction and return it to the Licensing Office. The licensee retains a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the VDSS web site within five (5) days, regardless of whether or not the Plan of Correction has been completed.

Violations:
Standard #: 22VAC40-131-250-M
Description: Violation: Based on record review and interview, the licensee failed to arrange a pre-placement visit for Foster Child 1 (FC1). Findings: The licensee received an application for admission for FC1 on 1/3/2019 and the child was placed. The circumstances of placement did not meet the definition of an emergency placement. Upon interview, S1 acknowledged the finding.

Plan of Correction: Executive Director and Program Administrator will train TFC staff on the definition of emergency placement per regulation.

Standard #: 22VAC40-191-40-C-1-d
Description: Violation: Based on record review and interview, licensee failed to conduct central registry searches for both applicants in FH2 in all states the applicants resided in during the last 5 years. Findings: The applicants in FH2 completed Virginia Central Registry Release of Information forms and Sworn Disclosures. The Virginia Central Registry Release of Information forms for both applicants documented that each lived in another state for a period that ended in May 2014. The home was approved on 4/24/2019. The case record did not contain central registry searches for the other and the Home Study report did not indicate requests were made for central registry searches in the other state. Upon interview, S1 acknowledged the finding.

Plan of Correction: Licensee will suspend family and conduct appropriate out of state central registry searches before reinstating applicant's approval. Senior Quality Assurance Program Administrator and Executive Director will be diligent in insuring all Central Registry Searches for applicants residing in other states in the last five years are conducted, reviewed prior to approval of the family
