An unannounced monitoring inspection was completed on May 09, 2019, from 10:57 a.m. to 3:02 p.m. for Circle Center Adult Day Care Services. Upon arrival, the licensing representative met with the designated staff member in order to initiate the entrance conference. The record review, encompassed ten(10) participant records and five(5) staff records. Participants were observed during the building tour involved in numerous activities. All prior cited violations were corrected and no new violations were cited during this monitoring inspection
