Comments:
An unannounced monitoring inspection was conducted on 5/31/19. At the time of entrance 92 participants were present with 18 center staff including the Director. This exceeds the required ratio of one staff per six participants. The sample size consisted of ten participant records and nine staff records. Participant interviews were not conduct as participants do not speak English and no interpreter was provided. Participant and staff records and other documentation reviewed. Criminal Background Checks of all staff hired since previous inspection were reviewed. Participants were observed eating and engaging in activities including group stretching, morning exercise, traditional Korean games, and Karaoke. Medication observation observed and violation notice issued and exit interview held. Description: Upon arrival participant was observed with a pill container out ready to administer medication to herself. While alert and oriented the Standard and Regulations were not meet that requires the medication be stored in a locked area or compartment and provided to the participant by staff upon request.

Plan of Correction: The Administrator will have a discussion with the participant to remind her of the policy surrounding the storage of medications.
